Root Canal Treatment

“Root canal” might be the two most dreaded words in dentistry. The reputation is decades out of date. Modern root canal treatment is closer to getting a filling than the ordeal most people imagine — and skipping it when you need one is almost always worse than doing it.

Dental pain has a way of making people negotiate with themselves. It’s not that bad. Maybe it’ll go away. I’ll see how it feels next week. Sometimes that works. With an infected tooth, it doesn’t — the infection doesn’t resolve on its own, and the longer it sits, the more complicated the situation becomes.

Root canal therapy exists to save a tooth that would otherwise need to come out. Extraction is always an option, but it’s not a neutral one — missing teeth affect bite, bone, and adjacent teeth over time. If there’s a chance to keep the tooth, it’s usually worth taking.

The patients who delay tend to regret it. Not because the procedure gets harder to do — it usually doesn’t — but because they spent weeks in unnecessary pain waiting for something that took about an hour to fix.

Inside every tooth, beneath the enamel and dentin, is a soft tissue called the pulp — a chamber containing nerves, blood vessels, and connective tissue. Root canal endodontic therapy is needed when that pulp becomes infected or dies, usually due to deep decay, a cracked tooth, repeated dental work on the same tooth, or trauma.

Once the pulp is infected, it can’t heal itself. Endo root canal treatment removes the infected pulp, cleans and shapes the canals, and seals everything so bacteria can’t re-enter. The tooth stays in place, functional and intact. Without treatment, the infection spreads — into the surrounding bone, potentially into the jaw and beyond. Dental abscess root canal treatment is often urgent for exactly this reason: an abscess doesn’t stay contained.

A dead tooth root canal treatment follows the same process — even if the tooth is no longer vital, it still needs to be cleaned and sealed to prevent ongoing infection.

Some of these are obvious. Some aren’t.

• Severe or persistent toothache — pain that doesn’t respond to over-the-counter medication or that wakes you up at night.
• Sensitivity that lingers — hot or cold sensitivity that stays for 30 seconds or more after the stimulus is gone.
• Swelling — in the gum near a tooth, in the jaw, or in the face.
• Darkening of a tooth — a tooth that’s changed color, particularly grey or brown, may have a compromised pulp.
• A pimple on the gum — a recurring pimple near a tooth is often a fistula, a drainage channel from an abscess.
• Pain on biting or pressure — a tooth that hurts when you bite down consistently.
• Tooth infection symptoms — fever, general feeling of illness, swelling that’s spreading.

Not every case is obvious. Some teeth need endodontic root canal therapy with minimal symptoms — which is another reason imaging matters.

1. Consultation and X-ray — digital X-rays, and sometimes a CT scan, show the extent of infection, the number and shape of canals, and whether there are complications like calcification therapy cases where canals have narrowed due to calcium deposits. Calcium hydroxide is sometimes used as an intracanal medicament in cases with significant infection, placed between appointments to further disinfect the canal system before final sealing.
2. Anesthesia — the area is fully numbed before anything else happens. Sedation is available for anxious patients. You will feel pressure but not pain.
3. Pulp removal — a small opening is made in the crown of the tooth. The infected pulp is removed using fine instruments, and each canal is cleaned and shaped.
4. Sealing — the canals are filled with a biocompatible material called gutta-percha and sealed. In some cases, a temporary filling is placed first with calcium hydroxide, and the permanent seal follows at a second appointment.
5. Crown placement — most teeth that undergo crown and root canal treatment need a crown afterward. The procedure weakens the tooth structure; a dental crown after root canal treatment protects it from fracture and restores full function. Anterior root canal therapy on front teeth sometimes requires only a filling if enough structure remains, but your dentist will advise based on your specific case.

The reputation comes from a time before modern anesthetics and techniques. Endodontic root canal therapy today is performed with fine rotary instruments, digital imaging, and effective local anesthesia. Most patients finish the procedure and say it wasn’t what they expected.

There’s mild soreness for a day or two afterward — the tissue around the tooth is tender post-procedure. Over-the-counter pain relief handles it easily for most patients.
